University of Alabama Passport Acceptance Facility

 

We are located in the Printing & Mail Services facility at 325 Kirkbride Lane (formerly known as 351 3rd Place).

Appointments are available Monday-Friday 9:00 a.m.-12:00 p.m.
Walk-ins are welcome from 12:00 p.m. - 3:00 p.m. (except for University holidays).

First Time Adult Applicants
Completed Application, Form DS-11 (new applicants) or Form DS-82 (renewals)
1. One passport photo – photos are available onsite for a fee of $15.00 (tax included) 2. Application Fee – check or money order only (amount varies depending on product(s) and services requested). Checks (personal, certified, cashier's, traveler's) and Money Orders should be made payable to "U.S. Department of State". Credit and debit cards CANNOT be accepted.
3. Execution Fee – Department of State mandated fee of $25.00; Money orders at all locations, payable as instructed by the facility, personal checks and cash (exact change only, credit cards)

5. Expedited Passports (new applicants only)

First Time Applicants 16 and 17 years of age
Most applicants age 16-17 must apply in person using Form DS-11.
Passports for applicants over 16 are valid for 10 years.
If you have your own identification document (ID), you may apply for a passport by yourself. However, we strongly recommend that at least one parent accompany you to show parental awareness.

Examples of Parental Awareness:

  • A parent appearing with you in person when you apply for your passport.
  • A signed statement consenting to issuance of a passport from at least one parent (should be accompanied by a photocopy of that parent’s ID).

If you do not have a current ID, you must show at least one parent knows that you are applying for a passport.
You (or your parent/guardian) must present one of the following IDs to the acceptance agent. 

  • In-state, fully-valid driver's license
  • Fully-valid, undamaged U.S. passport (may be expired)
  • Certificate of Naturalization or Citizenship (if the photo is still looks like you)
  • Government employee ID (city, county, state or federal)
  • U.S. military ID or military dependent ID
  • Valid foreign passport
  • Matricula Consular (Mexican Consular Identification, commonly used by the parent of a U.S. citizen child applicant)

1. Proof of U.S. citizenship - original or certified copy of his/her U.S. Birth Certificate, Certificate of Birth Abroad, or Certificate of Naturalization
2. Photo Identification - Driver's License, Government or State-issued ID card
3. One passport photo - photos are available onsite for a fee of $15.00 (tax included)
4. Application Fee - check or money order only, amount varies depending on product(s) and services requested.
5. Execution Fee - Department of State mandated fee of $25.00

First Time Applicants Under 16 years of age
All passport applicants under 16 must apply in person using Form DS-11.
Please note: Passports for children under 16 are only valid for 5 years.

1. Proof of U.S. Citizenship - original or certified copy of the child's U.S. Birth Certificate, Certificate of Birth Abroad or Certificate of Naturalization for one or both parents
2. Both parents, along with the child, must be present with photo identification (Driver's License, Government or State-Issued ID)
3. One passport photo - photos are available onsite for a fee of $15.00 (tax included)
4. Application Fee - check or money order only, amount varies depending on product(s) and services requested.
5.Execution Fee - Department of State mandated fee of $25.00

Renewing a Passport
Can I renew my passport online or in person? Passports can be renewed online or in person. If you meet the criteria for renewal, it is not necessary for you to apply in person to renew your passport.  If you are applying on Form DS-82, acceptance facilities will not take your application. You must mail in your renewal application yourself. Photographs
We will take your picture for your passport, here in our office, for an additional $15.00 fee. If you choose to bring your own photo, please be aware that all passport photos must be 2" x 2" and must meet these requirements. As of November 1, 2016, no eyeglasses are allowed in your passport picture.

Non Refundable Fees
The passport application fee and the execution fee are non-refundable. They are collected and retained by law even if a passport is not issued.
